  • Overview

    The Admission Intake Nurse  is responsible for timely pre-admission clinical review of potential patients, to include admission and re-admission screening tools, MARs and TARs, clinical and rehab notes, labs, and other necessary information. The clinical information will be provided through the regional care manager screens as well as electronic referral systems and hospital medical record. The Admission Intake Nurse will work directly with the DON or designee when questions arise regarding clinical complexity.   The Admission Intake Nurse will work side by side with the Admission Director to expedite admissions to skilled facility, determine appropriate bed for admission, and to confirm admission details.  Will communicate directly with Regional Care Managers and hospitals via phone, email and electronic referral system notes.  Will follow patients re-admitted to hospital for appropriateness to return to facility. May assist as back up to Case Manager for vacation or sick coverage. with a $$$$ Sign ON BONUS

    Duties and responsibilities of this position include, but are not limited to the following tasks and assignments as assigned by the facility admissions director:



    1.        Pre-Admission Procedure:
    2. Will conduct facility tours
    3. Check empty room for admission readiness.
    4. Assist Admissions Director with all pre admission paperwork.

    2. Assist the Admissions Director with coordinating admission with all parties, i.e. referring hospital, family, and facility departments to ensure a smooth transition for a resident.
    3. Assist the Admissions Director with completing all necessary documents for admission to include the contract, arbitration agreement, AOB, MSP, and other admission paperwork as deemed appropriate.



    III.       General Administrative:

    1. Abides by the standards identified in FutureCare’s Statement of Corporate Ethics and the Corporate Compliance Plan.
    2. Assist the Admissions Director with daily and monthly reports as assigned and required.


    1. Education: RN or LPN .
    2. Experience:  Minimum one year experience in Admissions, long-term care and/or sub acute care. Knowledge of ventilator and dialysis care preferred.  Strong oral and written communication skills, required.
    3. Physical Demands:  Involves sitting, standing, and walking most of the time; may also require driving.
    4. Travel:  Minimal
    5. General Requirements: Must be capable to execute all terms and conditions set forth in the FutureCare Health and Management Corporate Employee Handbook.
    6. Maintain a dependable attendance record and adhere to standards of cleanliness, grooming, hygiene, and professional dress code
